Work experience placement request form

Work experience placements are assessed on a case-by-case basis provided all conditions are met. (Please see Work experience request requirements). The City of Gosnells does not guarantee that a work experience placement will be offered upon completion of this form. This will be subject to staff availability to monitor and guide students during their practice. Once completed, this form will be forwarded to managers and coordinators of the relevant departments where you would like to undertake your work experience.

Please note: Fields marked with an asterisk are mandatory.

Work experience request requirements

• Only available to students (high school, TAFE, university or registered providers). Work experience is only for vocational purposes and subject to availability.
• Insurance must be provided by your educational institution.
• This form must be submitted at least four weeks before the expected start date.
